How is the 4C Diagnostic different from an engagement survey?
Engagement surveys measure sentiment. The 4C Diagnostic maps where perceptions diverge across teams and leadership levels on the conditions that drive execution—so you can pinpoint misalignment, prioritise what to fix first, and track whether alignment improves after action.
How quickly can we start
After a short call to understand your context, challenges and aims, we can deploy the survey straight away. Once responses are complete, you receive board-ready insights within two business days.
How long will it take people to complete the survey
It takes 22 minutes per person. People can complete it on their phone via QR code or on a computer. This replaces lengthy, costly interviews while giving you clearer data.
Is the questionnaire anonymous
Yes. By design we do not collect employee emails, which helps people answer honestly and reduces overthinking. You will still get accurate results even without 100 percent participation.
If leaders want to track who has completed, we can add an email field only for completion tracking with clear communication. Results remain anonymous to leadership.
Can we use our own change or HR team

How do you handle data
Responses are hosted securely. Output to leaders is anonymised and aggregated, with privacy thresholds to protect individuals and small groups.
Can the assessment be customised
The diagnostic questions are standardised for reliability. The demographic lenses are customisable at additional cost. Common options include department, function, team, role or position, seniority, years of service, employment type, gender, age, ethnicity and, where appropriate, neurodiversity. We can add other lenses to match your needs, and we apply minimum group sizes to protect anonymity.
What makes 4C different from other tools
Most tools are either personality profiles or broad sentiment surveys. 4C is perception-gap based. It maps where views diverge across leadership levels and teams on decision making, communication, trust, information flow, accountability and adaptability. The question design uses indirect, scenario-based items to surface implicit perceptions and reduce response bias, so you see where gaps are largest, where they are smallest, and where to start. It is peer reviewed in the OD Network journal and takes 22 minutes to complete.
